Your Rights Under the No Surprises Act
Under the No Surprises Act (effective January 1, 2022), you have the right to a Good Faith Estimate (GFE) of expected charges before your procedure.
- If you are uninsured (self-pay), or are choosing not to use your insurance for a service, we are required to provide a written Good Faith Estimate before your scheduled service.
- The GFE covers expected facility charges. Note that your surgeon and anesthesiologist bill separately and are not included in this estimate.
- If your final bill exceeds your Good Faith Estimate by more than $400, you have the right to dispute the charges.

What to Bring

- ✓Referral or prior authorization number (if required by your plan)
Billing Information
Beth Israel Lahey Health Surgery Center Plymouth will submit a facility claim to your insurance. Your surgeon and anesthesiologist bill separately from this facility.
Your Responsibility
Your out-of-pocket costs (copay, deductible, coinsurance) depend on your specific plan and benefits. You will receive a statement after your claim is processed.
