How to Refer a Patient

Once we receive the referral, our pre-op coordinator will contact the patient within 1–2 business days to schedule the procedure and coordinate pre-operative testing and clearance.

Spine
- Lumbar microdiscectomy
- Anterior cervical discectomy and fusion (ACDF)
- Lumbar laminectomy / decompression
- Lumbar fusion (TLIF / PLIF)
- Cervical laminoplasty
Patient Criteria
- ASA physical status I, II, or select III
- BMI generally 40 or below (evaluated case-by-case)
- Stable comorbidities, appropriately managed
- No high-acuity cardiac or pulmonary conditions requiring inpatient monitoring
- Responsible adult available for discharge
What to Include with the Referral
- ✓Patient name, date of birth, and contact information
- ✓Diagnosis and planned procedure (CPT code if available)
- ✓Relevant imaging reports (X-ray, MRI) or image access instructions
- ✓Current medication list
- ✓Relevant medical history and comorbidities
- ✓Insurance information and pre-authorization status (if applicable)
- ✓Your office contact information and preferred communication method
After the Referral
Patient Contacted
Our scheduling team reaches out to the patient within 1–2 business days to confirm the procedure and schedule a date.
Pre-Op Coordination
Our pre-op coordinator arranges required medical clearances, pre-operative testing, and instructions for the patient.
Schedule Confirmed
Procedure date and arrival time confirmed with patient and referring office. Pre-op instructions sent directly to patient.
Operative Report
A procedure note and operative report are sent to your office after the procedure is completed.
